Life and Accidental Death & Dismemberment (AD&D)
Life insurance is paid out at two-times the annual salary, not to exceed 500.000 USD. Double indemnity is paid for AD&D. This is a company-paid plan.
Supplemental Life
Employees may purchase a portable supplemental life insurance policy for themselves, spouses, and children. A monthly premium is paid via payroll deduction. There is a guaranteed issue of a 100,000 USD policy upon hire. The plan allows employees to purchase up to fives times their annual salary after successful completion of a health questionnaire.
Short-term Disability
Non-exempt employees are provided company-paid insurance at 60% of base salary plus shift differential, if applicable, when an employee is diagnosed by a physician as unable to work for a non work-related illness or injury.
Exempt employees are covered under Salary Continuance.
Long-term Disability
All employees are provided company-paid insurance at 60% of base salary for non work-related illness or injury longer than 90 days.
Long-term Care
Employees may purchase a portable long-term care insurance policy for themselves, spouses, parents, and parent-in-laws. Rates are based on age.
Workers' Compensation
This is a company-paid insurance program to protect the employee's income because of a work-related illness or injury while unable to work.
